About Us

IlliniCare Health Plan is….

Part of the Illinois’ Integrated Care Program. We serve about 20,000 members in suburban Cook, DuPage, Lake, Kane, Kankakee and Will counties. Members of the Integrated Care Program are older adults and adults with disabilities that qualify for Medicaid but are not eligible for Medicare.

Focused on health. As a health plan, our main goal is to provide our members with the best care. We want our members to achieve the highest possible levels of wellness, independence and quality of life. We do this by partnering closely with our providers so all members have a medical home. We have high standards of care, and ensure those standards are exceeded. We educate our members on preventive health. We work with community organizations to ensure our members have a strong network of support. We connect members to resources so they can get the extra help they need. We have a dedicated staff that works one-on-one with members to help them achieve their health goals.

Staffed by local, passionate employees. IlliniCare is comprised of local staff members, who are devoted to improving the health of the people we serve. Our staff is made up of doctors, nurses, behavioral health case workers and social workers. We employ the experts, because our members deserve the best. They have the experience and knowledge to help coordinate care for our members. They work to ensure our members get the best service and care to improve their overall health.

Dedicated to our community. IlliniCare is committed to doing more in the community we serve. We want to improve not only our members’ health, but the communities in which they live. Our staff-run program, the H.O.P.E Program (Helping Other People Everywhere) is one example of how IlliniCare employees work together to help the community.
